Rear trailing arm rust, how concerned should I be?

Not looking good.
Would not expect any manufacture to a thing about rust on a 19 yr old vehicle, driven in the salt belt. I've seen 5 yr old frames rotted in half, GM does nothing but try to sell a new truck.

This is one of the biggest issues with Elements. Honda really went cheap on the rust proofing (or lack thereof).
Rust happens to every vehicle driven up here in the rust belt. It's a good reason to lease if you live in those areas. If you plan to own and keep it for a long time, yearly or biyearly of fluid film or similar coating is well worth the expense.
Those of you in FLA, need not comply LOL.
